What You'll Do:
Partner with the Case Manager to provide skilled SLP services to patients in their homes.
Evaluate and treat speech, language, cognitive, and fluency disorders.
Educate patients and their families about communication strategies and techniques.
Serve as a patient advocate, ensuring their needs are met.
Collaborate as a valuable member of the interdisciplinary care team.

What You'll Need to Succeed:
Current and unrestricted CA Speech Therapist License.
Current BLS/CPR Certification.
Valid CA Driver's License.
Knowledge of individual development, human behavior, and the influence of culture on healthcare.
Knowledge of state and federal homecare regulatory guidelines (preferred).
Experience in home healthcare (highly preferred).
Experience with high-acuity patients (a plus).
Experience working with an interdisciplinary team (highly desired).
Case Management fundamentals (highly desired).
Self-direction, empathy, and critical thinking skills.
Outstanding interpersonal and leadership skills.
Strong organizational skills.
Our Hiring Process:
Prior to an in-person interview, candidates will complete a computer literacy and competency assessment to ensure you're set up for success in our tech-enabled environment. This assessment is confidential and will be used as one factor in our hiring decision.
Compensation:
$110,000 - $125,000 per year. Starting pay is commensurate with relevant experience. We also offer generous bonus incentives, comprehensive health benefits, and 401(k) with up to 4% matching!
